You can't get into windows in fail-safe mode? Then something is wrong so i wonder if you have been in contact with Samsung support for aid in troubleshooting?

I have checked the manual at Samsung and optimal is 1280x1024 60Hz and max 1280x1024 75Hz so try these and if the monitor don't work try it on another comp to rule out it's the monitor that is faulty or not.

try putting in a different graphics card,
That will force windows to start on minimum resolution,
and start solving your problem.
